Please use this form to register each swimmer in your family for Swimmer Room and Air. It will be a two-step process, as follows:

  1. Complete the form below to register your swimmer(s) for space at the Team Hotel.
  2. Click on "Complete Hotel Registration & Proceed To Next Step." Complete this second form and submit.

 

STEP 1: REGISTER SWIMMER FOR HOTEL ROOM

Complete the form below to register your swimmer(s) for space at the Team Hotel. You can also indicate with whom your child would like to share a room. Coach wants all swimmers to check-in to the Comfort Suites Columbus OH on Wednesday December 9th.

The total cost per swimmer is $550 and this includes hotel, ground transportation, meet entry fees, AND airfare. Swimmers will be reimbursed $500 from Florida Swimming and the Link to that is http://www.floridaswimming.org/EF/EF%2009/EF%2009%20Swimmer%20Support.pdf (instructions on how to complete the form will follow later). Please mail or drop-off your check payment so that it is in the hands of Kate Skaggs by NOVEMBER 6th.
